2.MGIMS Wardha- Pleural effusion, Right side complete hemiplegia,Indirect hernia,LBW newborn, PEM.

3.St Stephens Delhi - LRTI, anaemia,TOP with pneumonia,enteric fever,enterocutaneous fistula.(Credit-Dr.Gayatri).

4.CMC Vellore-  TB+ HIV + Severe wasting + malnutrition + generalised lymphadenopathy+ right sided pleural effusion+ alcoholic liver disease+family conflicts + mild depression
DM Type 2 + multiple injuries +staying alone in house
3 days old normal newborn
7 year old boy with second episode of partial seizure with  fever.
Right side inguinal hernia direct type without any obstruction or strangulation

How study orthopaedics

Dear friends,
        Orthopaedics is a vital subject in D.N.B. final theory paper 2 and we only get 1 month posting in ortho. So its very difficult to master all basic  aspects of the subject in short time.You can prepare theory from few U.G. time books like Maheshwari or others if any you know.But working with your orthopaedic surgeon consultants is important.
                 Observe how they diagnose ,investigate,manage,counsel their patients.How they correlate their management with other surgeons or physicians when multiple co morbidities are their in a patient.You can also brush few skills in casualty posting like Thomas splint application,slab preparation,managing trauma.Its interesting subject to learn in short time.The aim of this posting is that you should be able to diagnose patient in time and refer them in time to right centre.Learn to master aspects of conservative orthopaedics too.Its not always about C arm and O.Ts. PG- admission blues: Tips for FM-seekers.

Gone are the days when those who got poor ranks in PG-CETs used to opt for FM.
Over the last couple of years, in most good institutes, FM seats disappeared well before conventional specialities such as surgery, medicine, anesthesia etc in average places.

However, FM is not for everyone.

1. Do NOT take FM simply because you are not getting anything "better" .
Read up about FM on the net. FM has a lot of strengths and a few boundaries. Understand the concept. Depend on advice from those who KNOW about it, not from those who know nothing about it. Be careful who you seek advice from. Make an informed choice.

2. Do NOT take FM under the mistaken belief that it is "light" training.
In fact due to the vast scope of FM, it becomes crucial that you see as many patients as possible, spend as much time with teachers as possible, and know as much of real-life applications of medical knowledge as possible. In simple terms, it demands more efficient learning.

3. An odd FM-trained person here or there sporadically gives negative views about FM as a specialty.
The usual story in such cases is that - the person failed as a "person", and the specialty was never to blame. Not every Neurologist/Cardiologist etc is a smashing success; the same applies to FM too.
The problem with the the odd person who "failed" or "left" FM is that such people fail to look "within" and need something to deflect blames on. In all likelihood, such people never passed FM finals due to various reasons and "warn" others accordingly.
So again- be watchful. Seek balanced, unbiased opinions.

4. State/ National level Family Medicine CMEs are held every 6-12 months somewhere or the other.
Attend and interact with seniors to get a clearer idea what it is all about and what all is going on world-wide in the field of FM. While at it, don't be surprised with the strong, ubiquitous presence and progress of FM in several developed Nations.

5. Outside the syllabus, FM training demands a good attitude, willingness to learn from everything and everyone (humility), a highly logical thought process, ability to see, analyse and manage a variety of scenarios in practice, several managerial qualities, empathy, communication skills, patience and an open mind- some of these are traits which can be done away with in several other specialties, which need different personal aptitudes and skill sets.

As a career, FM is awesome; let there be NO DOUBT about this.
But if FM doesn't suit you as a person, better NOT take this .

Looking back, for example, the movie "Munnabhai MBBS" was all about Family Medicine! The movie exploited the public's desire for the need for doctors with technical + apt humane qualities, which was the basic message of the popular movie.

Scene: Dissection hall; Munnabhai, to the Dean:
" Dekh, apun ko na, ek "full-to" Doctor banneka hai. Specialist nahi- ke gale ke Doctor ko pata nahi, ke ghutne me kya chalrela hai".

Remember the cheering and whistling in the theatre to this dialogue?

If you love that kind of concept, go ahead, and join.
Otherwise, better look for something "better".

Is geriatric medicine a good choice post D.N.B. family medicine?

    In recent time few D.N.B. family medicine aspirants and few D.N.B. family medicine residents asked me  how about geriatric medicine course post family medicine? To be honest I myself did a course after my family medicine degree last year and I am expecting a degree by university soon if I mange to pass exam  gave last month.I did my course as distance education via NIMS university Rajasthan. It was 1 year course all self learning.I opt to do it within  6 months time and I must share it was not so tough as I have worked lot with geriatric problems in my residency days.I use CMDT geriatrics and textbook of geriatrics by Dr.Gupta who is associated with geriatric society India.Other than this course IGNOU also has a course on geriatrics which D.N.B. is recognizing as entry point to D.N.B. family medicine secondary.About course I am not so sure but you can get information about it from IGNOU study center near you.Geriatric courses as distance education is a good move as in your practice you face lots of geriatric patients and there problems.These kind of courses also help you to get attachment to old age homes which are on rise in cities and metro.Chennai has few geriatric coursed of various duration but they are full time courses.One boy ask me can we do simultaneously with D.N.B.Well you can but will be a gamble and hectic thing as residency as D.N.B. is hectic job.so if you are sure you can balance both and you institute is not having problem you can.So all the best friends who want expand there horizon as gerontologist with primary care physician approach. all the best.Almighty bless us all.:-).

How to prepare for practical D.N.B. family medicine

Dear friends,
           Few of you asked me mam how to prepare for practicals as our results are on the way and we may get max 3 weeks time in between results and practical dates?
           As  many of you are balancing last few days of residency & work simultaneously,basically you need to be focused while preparing for pracs.Divide your days or any format your are comfortable with and do daily bedside presentations to your seniors,juniors,consultants and if  no one available to mirror.:-).
      Read Kundu in detail both volumes for medicine as two cases there which you need to finish in 20 mins each.Read Das clinical for surgery cases .Read Julius Scott and Santosh Kumar for pads as again two case will be there.Learn all signs,d/ds,Demonstrations well.Read park selectively and obs esp for contraception part.All national programmes ,vaccines,E.C.G.s ,X Rays ,common drugs,instruments you should know.Its an easy exam if you have worked well in wards.Keep family physician in you awake while answering so they may like your approach and confidence.So all the best.let Almighty bless us all.All the best.:-).
